Mission Statement 
The Mission of the Community Transition Council of Hamilton County is to empower individuals as they prepare today for their lives tomorrow by coordinating or creating resources and opportunities within the community to achieve personal, societal and career goals.


About us
The Community Transition Council of Hamilton County is a liaison between the community and individuals with disabilities.  By helping identify vocational, leisure and residential opportunities in Hamilton County, the Council is creating a more productive, diverse community.

The Council's membership consists of young adults with disabilities and their families, state agencies, private service organizations and the business community.  By building public awareness and tapping into community resources, the Council is helping individuals overcome the barriers which block full community involvement.

  • The Community Transition Council helps individuals:

  • Access independent living, recreational and transportation resources

  • Attain vocational training and employment opportunities

  • Locate alternative educational opportunities related to individual needs

  • Acquire information regarding long-range financial and person centered planning

The Community Transition Council helps businesses:

  • Utilize Hamilton County community resources

  • Access a pool of qualified workers

  • Coordinate job training and employer expectations

  • Develop an awareness of the advantages related to employing individuals with disabilities
