Aktivate (Formerly Register My Athlete) is now open for the 2024-2025 school year. Please upload a physical dated after April 1st, 2024 and complete the required consent forms.  There is no need to submit hard copies to school. Please find the instructions in the link below and contact Darren Monke with any questions.

IHSAA Physical Form

All forms must be completed, uploaded, and accepted prior to participation in Carmel Middle Athletics. All Physical Forms must be completed after April 1, 2024.  

Fall Sports
Interested in Football, Cheerleading, Volleyball, Cross Country or Tennis?  Tryout dates, schedules, callout information, and coaches’ email contacts can be found on our Athletics Webpage.

If interested in determining eligibility for free lunch/textbook assistance, an application must be submitted via the Free or Reduced Lunch link. For additional service or help in completing the form, you may contact our Student Services Coordinators.

If your child has received vaccines within the last year, state regulations require the vaccination records be submitted by the start of the school year. You may fax the records to 317-571-4067 (ATTN: Nurse Michelle McKeown) or email the records to mmckeown@ccs.k12.in.us. The State of Indiana now requires the Tdap, Menactra, and the 2nd chickenpox vaccines be administered by the start of a student’s 6th grade school year. If your child will not be receiving these vaccines due to medical or religious reasons, please complete and submit a medical or religious exemption to be filed on your child’s behalf.

Parents/Guardians are encouraged to pre-pay student lunch accounts online using the EZSchoolPay system, visit Carmel Clay Schools Food Services for more information. Cash will only be accepted in one serving line per cafeteria.

Book fee information will be sent out later in the Fall. You may pay book fees online with a credit card.

If there have been changes to your student’s medication information, please update those forms through your parent PowerSchool account. If there have not been any changes, you do not need to resubmit the information.

Students will be required to wear the provided CCS Wellness t-shirt this year.  Students can choose what is most comfortable for them in regards to pants/shorts.  All students will need to be sure to bring closed toe, laced and tied athletic shoes to wear for wellness class. Shoes such as “Crocs'' or “Birks” are not permitted due to student safety.

Schedules and locker information will be emailed to you prior to school starting. Please remind your child not to share the locker combination with anyone.

The picture that is taken at Orientation will be used for your child’s ID and yearbook. Photo packages need not be purchased for your child’s photo to appear in the yearbook. When placing an online order before you receive your student’s schedule, you may leave the teacher name blank. Please dress according to the school dress code for your pictures – no sleeveless tops or shirts that expose the shoulder or underarms.

Spirit wear can be ordered online!

Order 24-25 CAM Spirit Wear 

Each student will receive a student ID that they must have with them at all times during the school day.  Student IDs will be used to pay for lunch, check in to the front office when arriving late to school, checking out books in the media center, visiting the nurse and more.  Replacement IDs are $5 and can be purchased in Student Services.

Supplies are provided for students. A fee will be added as a part of material fees sent out later in the Fall.

To update your phone numbers, email addresses and emergency contacts, please do so in your PowerSchool account. To change your address, please present documentation of the new address to the office.

Yearbooks may be purchased at registration or online at yearbookordercenter.com and our school code is 8127.
