Students and parents, we are excited to welcome you to Carmel Middle School! Please begin enrollment by completing the Carmel Clay Schools Online Registration Packet. After completing the registration, we'll be able to schedule classes and prepare for the first day at Carmel Middle.

The Middle School Program of Studies outlines the courses available for 6th-8th grade students. It also explains the block schedule and scheduling timeline.



You can print the physical form from the Carmel Middle School website and return it to the school when completed. The consent and concussion acknowledgement are also available for completion through your PowerSchool account. All physicals and the consent and concussion acknowledgement form are due one week before tryouts/practice begins.

Book fee information will be sent out later in the fall. You may pay book fees online with a credit card.

Free and Reduced Lunch

If you believe you qualify for free and reduced lunch, please fill out the form available on the district website and submit it at registration. For additional service or help in completing the form, you may meet with our Student Services Coordinators during registration.


If your child has received vaccines within the last year, state regulations require the vaccination records be submitted by the start of the school year. You may fax the records to 317-571-4067 (ATTN: Nurse Michelle McKeown) or email the records to The State of Indiana now requires the Tdap, Meningococcal, and Hep A vaccines be administered by the start of a student’s 6th grade school year. The Hep A vaccine is a new requirement for the 2018-19 School Year, and applies to students in K-6th, and 12th grade. It will gradually be phased in to include all grades, so if you have students in 7th-11th grades, please consult your physician on starting the series. Please note that the Hep A vaccine is a 2 part series, with 6 months between the 1st and 2nd dose. Please send in documentation even if you have not completed the series. If your child will not be receiving these vaccines due to medical or religious reasons, please complete and submit a medical or religious exemption to be filed on your child’s behalf.

Lunch Account

You may prepay your child’s lunch account at registration. Meal prices are breakfast $1.35 and lunch $2.65. The online meal prepayment service is called EZSchoolPay.

Medication Permission / Student Health

If there have been changes to your student’s medication information, please update those forms through your parent PowerSchool account. If there have not been any changes, you do not need to resubmit the information.

PE / Wellness Uniforms

Uniforms may be purchased in the bookstore during the school year. 6th grade PE uniforms (shorts, t-shirts, and monitor strap) will be charged with book fees and will be handed out during Registration or the first week of school.

Schedule & Locker Combination

These will be available at registration. Please remind your child not to share their locker combination with anyone.

School Pictures

The picture that is taken at registration will be used for your child’s ID and yearbook. Order forms will be available at registration or you may order your portraits online.

Photo packages need not be purchased for your child’s photo to appear in the yearbook. If placing an online order before you receive your student’s schedule, you may leave the teacher name blank. Please dress according to the school dress code for your pictures – no sleeveless tops or shirts that expose the shoulder or underarms.

Supplies are provided for students. The only item we ask that you provide for your child are ear buds that can be plugged into devices here in the building.

Updating Contact Information

To update your phone numbers and/or e-mail address, please do so in your parent PowerSchool account. To change your address, please present documentation of new address to the office.
