School nursing, a specialized practice of nursing, protects and promotes student health, facilitates optimal development, and advances academic success. School nurses, grounded in ethical and evidence-based practice, are the leaders who bridge health care and education, provide care coordination, advocate for quality student-centered care, and collaborate to design systems that allow individuals and communities to develop their full potential.

Medication Policy

Medication Forms Required: Parents must complete the appropriate medical forms in PowerSchool before any medication or medical procedure can be administered at school. These include the Medication Intake Form, Medication Return Form, and Student Procedure Form, all available under the Medical section in PowerSchool (App or web). Forms can be submitted in advance or completed during clinic visits.
Transportation and Storage: All medication—prescription and non-prescription—must be delivered to and picked up from the school clinic by a parent or guardian. Students may not transport or possess medication unless authorized under emergency self-carry (see item 7).
Original Container: Medication must be in its original container. Prescription medication must include a pharmacy label with the student’s name, medication name, and administration instructions. Non-prescription medications must be labeled with the student’s name.
Permission to Administer: Medication must be accompanied by a current prescription label or written physician’s order. In addition, a parent or guardian must complete the Medication Intake Form in PowerSchool to authorize administration. Physician orders may be faxed directly to the school clinic.
As-Needed Medications: With proper documentation and parent consent, nurses may administer Benadryl (for allergic reactions such as bee or wasp stings, per weight guidelines) and over-the-counter pain relievers (e.g., generic Tylenol or Advil). Parents of elementary students (K–5) will be notified if an analgesic is given.
Medical Procedures: If a student requires a medical procedure during the school day (e.g., catheterization or tube feeding), parents must complete the Student Procedure Form in PowerSchool.
Emergency Self-Carry Medications: Students with chronic conditions may carry and self-administer emergency medications (e.g., inhalers, EpiPens) only with an annual written authorization from both the physician and parent. Documentation must be submitted through PowerSchool.
Medication Changes or Termination: If a medication is discontinued, changed, or no longer needed at school, parents must update the Medication Intake Form in PowerSchool to reflect the change. For medication pick-up, the Medication Return Form must also be submitted.
End-of-Year Medication Disposal: Any medication not picked up by the last day of school will be safely discarded. To retrieve medication, a parent or guardian must complete the Medication Return Form in PowerSchool.

Medical Form Instructions (PowerSchool)

Parents dropping off medication at school must complete the Medication Intake Form on PowerSchool. This form can be completed on the PowerSchool App or web-based platform. Parents may complete the form ahead of medication drop off or be instructed to complete the form while in the clinic at the time of drop off.

Select Student and then Forms.

Scroll to the Medical section and select Medication Intake Form.

Complete the form.
Note: The user can include decimals in the dosage numbers if necessary.

Select Submit when the form is complete.

For Medican Return Form or Student Procedure Form, follow same starting instructions and select the needed form to complete.

Immunization Information

Indiana law states that students in all grades are required to meet the minimum immunization requirements. The immunization record must include the student’s name, date of birth, and the vaccine given and date (month/day/year) of each immunization. Below lists the Indiana school requirements of vaccines with number of doses students must have before attending class.

Proof of immunizations must be submitted to your child's school nurse
by the first day of school.

THE CHILDREN AND HOOSIERS IMMUNIZATION REGISTRY PROGRAM (CHIRP)

The Children and Hoosiers Immunization Registry Program is an online system maintained by the Indiana State Department of Health that stores and updates immunization records of both children and adults in Indiana. This registry provides an excellent method of tracking student immunization history, and enables school nurses and others to easily check on the immunization status of students. The registry is currently available to the Indiana State Department of Health, the immunization data registry of another state, a healthcare provider, a local health department, an elementary or secondary school that is attended by the individual, a child care center, and the office of Medicaid policy and planning or a contractor of the office of Medicaid policy and planning. I.C. 16-38-5-3. Engrossed Senate Bill 219 would add a provider’s designee, a child placing agency, and a college or university that is attended by the individual to the list of persons to whom the immunization information may be released.
